New Patient Registration

If you live in our practice area and would like to register with us, please complete one of our registration forms that are available from reception.

On the form you will be registering with a GP but it is our policy that you can see any doctor.

Photographic identification (such as a drivers licence) and proof of address is needed for anyone registering aged 18 years and over; for children under the age of 18 photographic ID is needed if available, if photographic ID is not available a birth certificate will suffice.

You will also need to supply the NHS numbers of any person you are registering - this is given on your medical card if you have one or it can be obtained from your previous GP.

Patients with Particular Needs

Our surgery is accessible to patients using a wheelchair. We also have 2 parking spaces outside the practice which are reserved for patients displaying a disabled sticker.

We have a hearing loop in the surgery - please let us know if you have a problem which affects your ability to communicate.
